United Way of Northern Ozaukee is committed to delivering a more comprehensive and coordinated approach to health and human services for our most vulnerable and at-risk populations. For the second consecutive year, UWNO has reinvested your dollars to support the Community Services Navigator (CSN) initiative. The CSN, a low-barrier approach, swiftly connects individuals to the services they need to regain stability and thrive. A key feature of the CSN is its ability to create a customized plan of care tailored to the participant's needs, ensuring a personalized approach to each individual's situation. The CSN also facilitates access to necessary services, including medical providers, mental health services, substance abuse treatment, and various other community services, empowering individuals to take control of their lives.
In 2021, United Way of Northern Ozaukee began funding the newly created CSN position with its Community Impact Grant. The proceeds from the fall golf outing supported the Shared Care Coordinator resource, a key component that provides comprehensive care to families and individuals in our community. This resource, made possible by your support, is a testament to the collaborative nature of our work and its impact on the community. Advocates of Ozaukee, Lakeshore Regional Child Advocacy Center, Ozaukee Family Services, and Sirona Recovery connect interagency clients to the services and resources they need to reduce their risks and improve their overall well-being.

CSN Vision
